1. Information on how to enter and prizes form part of these Conditions of Entry.

2. Entry is only open to Australian residents aged 18 years or over.

3. If you are completing this survey for another individual, you warrant that you have sought and received their permission, and that they agree to be bound by these Terms and Conditions and the Privacy Policy of The Acuity PM Behavioral Index Survey. 

4. Promotion commences 18/08/09. Entries close 11:59pm EST on 15/10/09. Draws will take place on 16/10/09 at Level 2, 99 Queen Street, Melbourne Victoria. Winners will be notified by email and their names will be published on the Acuity Consulting Group website.

5. Total prize value is $ 5,000

6. Prizes do not include any travel or other costs incurred.

7. Prizes are not redeemable for cash or any other Acuity Programs / Services.

8. Acuity PM Behavioral Index Survey may conduct such further draws at the same place as the original draw as are necessary at 3:00pm on 01/12/09 in order to distribute any prizes unclaimed by that date. Winners will be notified by email and the names of the winners will be published on the Acuity Consulting Group website.

9. Prizes must be used by 30 March 2010.

10. Attendance of the preferred program date is subject to availability, where programs are fully booked applicants are able to apply for the next suitable program.

12. The prize definitions are as follows:

• Personality Profile report:
  Revealing  how your personality characteristics support or hinder the demonstration of the behavioural attributes of superior PM’s and includes a 1 hour feedback session  (Valued $700)
• Selection report:
  A comprehensive assessment of 2 candidates, detailing and comparing strengths and weaknesses to assist with hiring decision. (Valued at $2,500)
• Development program:  
  One place on Acuity's Touchstone Program - an introduction to Acuity’s full PM Behavioural Blueprint, using critical incident simulations in a safe learning environment. Program duration is 1.5 days ( Valued at $1,800)
